If I want to indicate that my sentence is asking a question, I think
its reasonable to terminate the sentence with ?
If I want to indicate that my sentence is expressing an exclamation, I
think its reasonable to terminate the sentence with !
If I want to indicate that my sentence is asking a question AND
expressing an exclamation...

Surely a question can be exclaimed? After all the ! is just indicating
the tonality and voicing of the spoken phrase.

its only really an issue in declamatory first person text (like
Usenet) - if you were writing fiction you wouldnt need the combined
exclamation/question mark. In fact, arguably, the exclamation mark
isnt strictly necessary in fiction at all.
